The UF Behavioral Health Hub, or BHH, Roadmap is comprised of these 7 steps:
- Your child’s Primary Care Provider, or PCP, asks the UF BHH for help and you will be asked to fill out paperwork at the doctor’s office or after speaking with the UF BHH Care Coordinator
- If your child is between the ages of 14-21, he or she will be asked to complete an Health Care Transition, or HCT, survey related to transition to an adult health care provider
- We will need the following: PSC-17, other clinical assessment tools as requested by your child’s PCP or by the UF BHH psychiatrist, Mental Health Background Information form, Consent & Authorization form, and a Release of Information form
- Your child’s PCP will talk to the UF BHH Psychiatric Provider team
- You and your child may be asked to join an appointment by Zoom or in-person
- Your child’s PCP will talk to you about the advice given by the UF BHH team
- In a few months, you and your child will be asked to complete another PSC-17 and a satisfaction survey
